Giving - Automatic Withdrawal Form

Fill out this form if you would like your giving to be automatically withdrawn from your bank account one or more times a month.
How it Works
1. Fill out the form below.
2. You will be able to choose if you want the whole amount taken once a month or divided evenly and taken at multiple times during the month.
3. Attach a picture of your void cheque.
4. In the event you switch bank account or need to alter the amount of the withdrawal please notify Soledad at treasurer@southwestchurch.ca before the 25th of the month.
5. A tax reciept will be issued at the end of the year, whether you give by automatic withdrawal or by another means.
